    Shamba Maisha: A pilot study assessing impacts of a micro-irrigation intervention on the health and economic wellbeing of HIV patients

    <p>Abstract</p> <p>Background</p> <p>HIV/AIDS negatively impacts poverty alleviation and food security, which reciprocally hinder the rapid scale up and effectiveness of HIV care programs. Nyanza province has the highest HIV prevalence (15.3%), and is the third highest contributor (2.4 million people) to rural poverty in Kenya. Thus, we tested the feasibility of providing a micro-irrigation pump to HIV-positive farmers in order to evaluate its impact on health and economic advancement among HIV-positive patients and their families.</p> <p>Methods</p> <p>Thirty HIV-positive patients enrolled in the Family AIDS Care and Education Services (FACES) program in Kisumu, Kenya were provided a micro-financed loan to receive an irrigation pump and farming guidance from KickStart, the developer of the pump. Economic data, CD4 counts, household health and loan repayment history were collected 12 months after the pumps were distributed.</p> <p>Results</p> <p>Mean annual family income increased by $1,332 over baseline. CD4 counts did not change significantly. Though income increased, only three (10%) participants had paid off more than a quarter of the loan.</p> <p>Conclusions</p> <p>We demonstrated the feasibility of an income-generating micro-irrigation intervention among HIV-positive patients and the collection of health and economic data. While family income improved significantly, loan repayment rates were low- likely complicated by the drought that occurred in Kenya during the intervention period.</p

    Hybrid Volitional Control of a Robotic Transtibial Prosthesis using a Phase Variable Impedance Controller

    For robotic transtibial prosthesis control, the global kinematics of the tibia can be used to monitor the progression of the gait cycle and command smooth and continuous actuation. In this work, these global tibia kinematics are used to define a phase variable impedance controller (PVIC), which is then implemented as the nonvolitional base controller within a hybrid volitional control framework (PVI-HVC). The gait progression estimation and biomechanic performance of one able-bodied individual walking on a robotic ankle prosthesis via a bypass adapter are compared for three control schemes: a passive benchmark controller, PVIC, and PVI-HVC. The different actuation of each controller had a direct effect on the global tibia kinematics, but the average deviation between the estimated and ground truth gait percentage were 1.6%, 1.8%, and 2.1%, respectively, for each controller. Both PVIC and PVI-HVC produced good agreement with able-bodied kinematic and kinetic references. As designed, PVI-HVC results were similar to those of PVIC when the user used low volitional intent, but yielded higher peak plantarflexion, peak torque, and peak power when the user commanded high volitional input in late stance. This additional torque and power also allowed the user to volitionally and continuously achieve activities beyond level walking, such as ascending ramps, avoiding obstacles, standing on tip-toes, and tapping the foot.     The impact of liver disease: a leading cause of hospital admissions in people living vith HIV

    Background: This study reviews recent trends of HIV inpatient admissions over 5 Infectious diseases Units in Liguria, in 2012. Patients and Methods: Five infectious diseases Units in Liguria, Italy, collected data on inpatient HIV admissions from January to December 2012, including patient demographic, discharge diagnosis, CD4 Tcell count, viral load (VL) and combined anti-retroviral treatment (cART). Results: Rate of patient admissions per 100 years was 6.12 (number=257), in 62.6% (n=161) of admissions a VL under 50 copies/ml was observed. Furthermore, 86.4% (n=222) of admissions were on active cART. Median age was 49 years. Mortality rate was 10.2%. Hepatitis C coinfection occurred in 64.6% of patients (n=166). The most common diagnosis was infectious diseases (29.1%), respiratory diseases (16.6%) and neoplasms (15.%). Chronic HCV infection and its complications (cirrhosis and hepatocellular carcinoma) accounted for 31% of all discharging diagnosis. Conclusions: The majority of inpatients admitted during 2012 in our Units were on cART and virologically suppressed. The complications of hepatitis C coinfection have a major impact on mortality rates and hospitalization rates in Italy. According to these observations, the availability of new drugs for chronic hepatitis C imposes a further effort to improve the quality of life of our patients

    Generating and probing a two-photon Fock state with a single atom in a cavity

    A two-photon Fock state is prepared in a cavity sustaining a "source mode " and a "target mode", with a single circular Rydberg atom. In a third-order Raman process, the atom emits a photon in the target while scattering one photon from the source into the target. The final two-photon state is probed by measuring by Ramsey interferometry the cavity light shifts induced by the target field on the same atom. Extensions to other multi-photon processes and to a new type of micromaser are briefly discussed

    Safety and efficacy of cannabidiol-cannabidiolic acid rich hemp extract in the treatment of refractory epileptic seizures in dogs

    The use of cannabidiol (CBD) in childhood refractory seizures has become a common therapeutic approach for specific seizure disorders in human medicine. Similarly, there is an interest in using CBD, cannabidiolic acid (CBDA) or cannabinoid-rich hemp products in the treatment of idiopathic epilepsy in dogs. We aimed to examine a small cohort in a pilot investigation using a CBD and CBDA-rich hemp product for the treatment of refractory epileptic seizures in dogs. Fourteen dogs were examined in a 24-week randomized cross-over study being provided placebo or CBD/CBDA-rich hemp extract treatment at 2 mg/kg orally every 12 h for each 12-week arm of the study. Serum chemistry, complete blood counts, serum anti-seizure medication (ASM) concentrations and epileptic seizure frequency were followed over both arms of the cross-over trial. Results demonstrated that besides a mild increase in alkaline phosphatase, there were no alterations observed on routine bloodwork at 2, 6, and 12 weeks during either arm of the study. Epileptic seizure frequency decreased across the population from a mean of 8.0 ± 4.8 during placebo treatment to 5.0 ± 3.6 with CBD/CBDA-rich hemp extract (P = 0.02). In addition, epileptic seizure event days over the 12 weeks of CBD/CBDA-rich hemp treatment were 4.1 ± 3.4, which was significantly different than during the 12 weeks of placebo treatment (5.8 ± 3.1; P =0.02). The number of dogs with a 50% reduction in epileptic activity while on treatment were 6/14, whereas 0/14 had reductions of 50% or greater while on the placebo (P = 0.02). No differences were observed in serum zonisamide, phenobarbital or bromide concentrations while on the treatment across groups. Adverse events were minimal, but included somnolence (3/14) and transient increases in ataxia (4/14) during CBD/CBDA-rich hemp extract treatment; this was not significantly different from placebo. This further indicates that providing CBD/CBDA-rich hemp extract during refractory epilepsy (only partially responsive to ASM), in conjunction with other ASM appears safe. Based on this information, the use of 2 mg/kg every 12 h of a CBD/CBDA-rich hemp extract can have benefits in reducing the incidence of epileptic seizures, when used concurrently with other ASMs

    The success of HIV combination prevention: The Dean Street model

    The 56 Dean Street combination prevention model, a strong engagement with the LGBTQI community and flexible services adapted to users’ changing needs led to an 80% drop in HIV diagnoses in gay, bisexual and other men who have sex with men (GBMSM) from 2015 to 2017. We describe the service changes at 56 Dean Street since 2012 which resulted in an increase in the frequency of HIV testing, the introduction of pre-exposure prophylaxis, earlier HIV diagnosis and a shorter time to viral suppression in those living with HIV. This model could be adapted to deliver similar results in those settings of high HIV prevalence among GBMSM and where access to technological innovation in healthcare and engagement with the community can be achieved
